Independent Health is requesting that your organization agrees and attests to the following requirements regarding Medicare training:

• My organization complies with CMS compliance and FWA training requirements.
• All workforce associates employed by my organization have completed training within 90 days of initial hire/contracting and annually thereafter.
• My organization maintains documentation of training completion of all workforce associates who take training upon hire and annually. Documentation includes training logs, system generation reports, spreadsheets or sign-in sheets. Furthermore, this documentation must include the employee's name, dates of employment, dates of completion, and passing scores (if captured).
• Such documentation is maintained/retained by my organization for a period of 10 years
• My organization acknowledges that Independent Health reserves the right to audit and verify my organization's training completion.
